Delta Communications, LLC dba Clearwave Fiber (herein, “Clearwave Fiber”), its affiliates, successors and assigns, are given the absolute right and unrestricted permission to use and exploit, and grant others the right to use and exploit (both electronically and in all other formats, whether now known or hereafter developed), your name, testimonial, biographical data, photograph, picture, voice, video recorded image or other likeness of you or of material you have created and have authorized Clearwave Fiber to use (collectively hereinafter, the “Images”), through any form of media (print, digital, electronic, broadcast or otherwise), for advertising, marketing, publicity, or any other lawful purpose, including websites, blogs, social media channels and advertising channels. You understand and affirm that you have the legal rights to grant Clearwave Fiber permission to use the Images and that the Images may be used in perpetuity. You waive any right that to inspect and approve the finished product that may be used or to which it may be applied now and/or in the future, whether that use is known to you or unknown. You further acknowledge that your participation is voluntary, and that you will not receive financial compensation of any type associated with the use or publication of the Images. You also acknowledge and agree that Clearwave Fiber’s use or publication of said Images confers no rights of ownership or royalties whatsoever. You hereby release and agree to hold harmless Clearwave Fiber, its contractors, its employees, and any third parties involved in the creation or publication of marketing materials or advertising from any reasonable expectation of privacy or confidentiality associated with the Images and from liability for any claims by you or any third party in connection with your participation or use of the Images. You certify that you are at least 18 years of age and that this release is signed voluntarily, under no duress, and without expectation of compensation in any form now or in the future.
